I do voluntarily submit my application for attendance and participation in the Martial Arts activity as shown on this form, and I assume full responsibility for any and all damages ,injuries or losses that I may incur, if any, while attending or participating, and I wave all claims for injuries against the promoters, or operators or sponsors of said Martial Arts activity individually or otherwise, for any claims for injuries I might sustain. I fully understand that any medical treatment given will be of a First Aid treatment only.

I consent that any pictures furnished by me or any picture taken of me in connection with the activity can be used for publicity, promotion or television showing, and I waive compensation in regards thereto.
